Overview
CareBridge's clinic partners coordinated patients across phone calls, fax threads and three inboxes. Details slipped; follow-ups slipped further. They needed one shared workspace the whole care team could trust.
I built it: patient timelines, appointment scheduling, task ownership and secure team messaging — with strict roles so each person sees exactly what their job requires.
Challenge
Healthcare workflows punish ambiguity. A missed referral or an unclear owner isn't an inconvenience — it's a patient waiting. The system had to make ownership explicit at every step.
Privacy shaped every decision: encrypted data in transit and at rest, audit trails, session controls and the principle of least privilege across the whole data model.
Approach
- Patient timelines unifying visits, tasks and messages
- Role-based access with full audit logging
- Scheduling with reminders that cut no-shows
- MongoDB schemas designed for clinical querying
- Zero-downtime migration from legacy records
Outcome
Care teams moved off the patchwork of calls and inboxes onto a single source of truth. Follow-ups have owners, handovers have history, and audits have answers.
The migration ran with zero downtime for customers — years of technical debt retired quietly while clinics kept seeing patients.
